How do I transfer my shares to another party?
Required Documents:
- Original Stock Certificate with a Medallion Signature Guarantee
- A Completed Shareholder Transfer Form
- Mailing Address for the New Certificate
- Check for the Applicable Fees
In the case of a decedent shareholder, we require the following in addition to the above:
- A Probate Court Document,
- A Copy of the Small Estate Affidavit,
- or Other Official Document
- A Copy of the Death Certificate

What is a medallion signature guarantee?
A medallion signature guarantee is a stamp provided by a bank or broker that verifies the signature on the certificate or stock power is that of the rightful owner. If you are no longer in contact with your original broker we can assist you with this.
